The finale defied expectations? What would have happened if KBS had decided to extend the series by a couple of episodes? Would the ending have changed?

Professor Kim decided not to have a second surgery and disappeared. He did not reappear again in the series, though Professor Lee imagined him; Kim was the voice of his conscience, or something. No real reconciliation between Kim and Lee, though Lee admitted that the completion of the research paper for which he was receiving a national award was all due to Kim.
Professor Lee had decided to take a position as a director at a rival university hospital, and he told Dr. Yun to apply for a fellowship position there. Instead, Dr. Yun told Lee that even though she likes, that is loves, him, she had accepted a fellowship at a university close to her home so she could be with family. She asked him to compromise and to wait for her. His response? He gave Yun an ultimatum -- either stay or forget about him waiting for her. So the viewer was then treated to 10 minutes of this playing itself out.
Surprisingly, she decided to stay, showing up for his award lecture. If this were an American drama, we would probably see him visiting her in her hometown (as she secretly desired, promising to herself that if he did so,
she would be his forever) and confessing his love to her. Or maybe he would send a MRI image of his brain, revealing his emotions to her. Or there is the extreme measure of going to her new hospital and working there. (But even Yun acknowledged that this was unlikely to happen, given his ambition.)
Initially, after he gave his ultimatum, he waited for her in the hallway, since on previous occasions she would run after him. But not that time, as she ate her instant noodles, crying, trying to remain strong. But he did win the waiting game, eventually. A rather "alpha" ending to the series, it seems to me.
